In this article, we would like to inform you of our new discharge report format to explain how we use the International Classification of Functioning, Disability, and Health (ICF) framework, Goal Attainment Scaling (GAS), and the Modified Barthel Index to drive our goals and outcomes, ensuring the best possible care for our patients.

3. Modified Barthel Index: Measuring Functional Independence
The Modified Barthel Index is a widely recognized tool for measuring a person’s level of functional independence in performing activities of daily living, such as bathing, dressing, and mobility.
