Measuring Excellence at Faircape Health Rehabilitation Centre

In this article, we would like to inform you of our new discharge report format to explain how we use the International Classification of Functioning, Disability, and Health (ICF) framework, Goal Attainment Scaling (GAS), and the Modified Barthel Index to drive our goals and outcomes, ensuring the best possible care for our patients. 

1. The ICF Framework: A Holistic Approach to Assessment

The ICF framework, developed by the World Health Organization, is a comprehensive model that helps us understand the impact of health conditions on a person’s functioning and disability. It encompasses three main components: 

Body Functions and Structures: Activities and Participation:  Environmental Factors:
We assess the physical and mental aspects of our patients’ health.We evaluate how these health conditions affect a person’s daily life and participation in society.We consider the influence of the physical, social, and attitudinal environment on a person’s functioning.

2. Goal Attainment Scaling (GAS): Personalised Goal Setting

Goal Attainment Scaling is a method we use to set personalised rehabilitation goals with our patients. Instead of relying on generic goals, GAS allows us to work with patients to define specific, achievable objectives that are meaningful to them. Each goal is assigned a numerical score, and we continuously monitor progress. This approach ensures that our rehabilitation plans are patient-centered and outcome-driven.

3. Modified Barthel Index: Measuring Functional Independence 

The Modified Barthel Index is a widely recognized tool for measuring a person’s level of functional independence in performing activities of daily living, such as bathing, dressing, and mobility. We use this index to assess and track our patients’ progress. By regularly assessing Modified Barthel Index scores, we can quantitatively measure improvements in our patient’s functional abilities.
